We are currently accepting new patients! See below for details.
The typical new patient visit at Lardi Chiropractic Clinic will last about an hour. Your health is very important to us and we want to have adequate time to assess your health needs as our services go beyond chiropractic care. See below for how to schedule and prepare for your appointment.
Call our office
Please call our office at 815-634-3750 to schedule your appointment. Leave a message if no one answers. We are often very busy with patients and will give you a call right back. Let our front desk staff know what services interest you and if there's a specific doctor you would like to see.

If you are interested in any nutritional or lifestyle recommendations please let the front desk staff know when you call to schedule. You may be asked to provide recent lab work and health records before seeing the doctor. This allows our doctors to make better use of your time during your first visit.

If you are making an appointment for a minor, please be aware that a parent or guardian will need to accompany them to this visit.
What to bring with you
Please bring a driver's license or State ID with you as well as any insurance cards you may have. If you have had recent X-rays (or CT or MRI), bring a copy of them on disc and/or any radiology reports.

If you have access to a printer, please print the below forms, fill them out, and bring them with you to your first visit.

If you are not able to print and complete the above forms, please arrive 15 minutes before your scheduled appointment time to fill out paperwork.
Cost and payment
Lardi Chiropractic Clinic is in-network with Blue Cross Blue Sheild and Medicare. If you have a different insurance, you may have out-of-network benefits. We are out-of-network with Aetna, Cigna, Humana, and United Heathcare. We recommend calling your insurance ahead of time to ask about your chiropractic benefits at Lardi Chiropractic Clinic.
We do have an insurance specialist on-site who will check your benefits for you as a courtesy, but time constraints may not allow her to give you specifics for your insurance plan before your appointment.

Plans vary greatly even within the same company according to the agreement you made with your insurance. We accept cash, personal checks, credit and debit card payments, as well as HSA cards.
